«Слететь с ручки». To fly off the handle — это значит внезапно сильно рассердиться, взорваться, вскипеть. Имеется в виду головка молотка, которая внезапно срывается с ручки во время работы, и работающий не имеет представления о том, насколько серьёзным может быть повреждение и кто может пострадать в результате этого.
